Sciatica refers to the radiating pain, numbness, or weakness that travels along the path of the sciatic nerve, which branches from the lower back through the hips and buttocks and down each leg. The condition is most often a symptom of an underlying issue, such as a herniated lumbar disc or a bone spur on the vertebrae, which compresses or irritates the nerve roots. Acute episodes of pain can be severe and debilitating, significantly affecting mobility and quality of life.
Initial treatment for sciatica typically focuses on conservative, non-surgical approaches. These include physical therapy (PT), which uses targeted exercises to strengthen core muscles, improve flexibility, and correct posture to alleviate pressure on the nerve. Other non-invasive methods involve applying heat and cold therapy, and using over-the-counter or prescription anti-inflammatory medications. For persistent or severe pain, localized treatments such as epidural steroid injections may be administered to reduce inflammation near the affected nerve roots.
When conservative treatments fail to provide relief, or in rare cases involving progressive neurological deficits like significant leg weakness or loss of bowel/bladder control, surgical intervention may be considered. Procedures like a microdiscectomy aim to remove the portion of the disc that is irritating the nerve. The overarching goal of all treatment modalities is to manage pain, restore function, and prevent recurrence through maintenance of a healthy spine and strong musculature.
